Application Process

1. Complete the application process for admission to Connors State College
   • Apply online at CSC Application (documentation of admission is required)
2. Obtain TWO official transcripts from ALL colleges attended
   • Send one copy to Connors State College for advisement jody.butler@connorsstate.edu
   • Send second copy to ICTC to complete application at AHCtranscripts@ictech.edu
3. Contact Jody Butler at Connors State College jody.butler@connorsstate.edu
   • Advisement & Degree Check
   • Obtain the OTA Academic Advisement form
4. Obtain additional documentation for points
   • Other documentation is required for awarding of points (See “Priority Point System” in the Advisement Packet)
5. Complete Observation Hours
   • Complete 24 hours of observation experience with an occupational therapist or occupational therapy assistant (required forms available in the Advisement Packet)
     • Minimum of 2 facilities, within the past year, with a variety of age ranges
     • Minimum of at least 8 hours at 1 of the observation sites
     • Cannot be at an applicant’s place of employment or with a family member
6. Complete application to OTA program at ICTC Application
7. Upload all required documents with the ICTC application or email to carol.williams@ictech.edu
   • Documentation of admission to Connors State College
   • OTA Academic Advisement form from CSC
   • Observation hours documentation
8. Submit official final transcript for spring semester (if applicable) to AHCtranscripts@ictech.edu by June 1
