If you have any technical issues, please submit a request for assistance to the CSC Helpdesk via email at email@example.com with the following information, at minimum:
- Name (First, Last, and Middle Initial plus any other names your account might be under)
- CWID Number (8-digit number that can be found on the back of your ID card)
- Birth Date (MM/DD/YYYY)
- Contact Information (email address, non-CSC email address if you cannot access your CSC email account, and phone number)
- Description of Your Problem (with any error messages you may have encountered)
